Countryside Health Care of Milford is seeking an enthusiastic and dedicated Rehab Desk Nurse to join our team.
Duties and Responsibilities:

Interpret and enforce facility policies and procedures

Effective communication and interaction with residents, families, physicians, consultants and other department personnel

Ensure necessary paperwork and documentation is completed for patient admissions, discharges, appointments, and overall short-term rehab stay
-
Evaluate patient needs and the effectiveness of care provided

Periodic chart reviews/audits
-
Book appointments, arrange transportation and prepare paperwork the day before appointments
-
Book stat labs, x-rays and other tests as needed and call for lab pick-ups as needed

Fill out lab slips per physician’s orders
-
Receive labs and report accordingly
-
Maintain universal precautions, infection control and safe work environment

Assist with quality assessment/performance improvement projects
-
Keep the director of nursing informed regarding the status of patient and staff issues
-
Assist with orientation and staff development
-
Participate in staff meetings and continuing education opportunities and communicate results of meetings to unit staff
-
Deliver direct resident/patient care when necessary
-
Assume supervisory responsibilities if requested by the DNS or designee

Perform other duties as assigned
Requirements:
-
Graduate of an accredited school of nursing with current Massachusetts license
-
Valid CPR certification
-
Able to read, write, speak and understand the English language
-
Excellent nursing, critical thinking and decision-making skills

Computer literacy
-
Excellent communication and interpersonal skills
-
Effective professional leader and role model
-
Basic knowledge of state, federal, OBRA, JCAHO, OSHA and facility policies and regulations
Salary:
$33 - $43 per hour based on years of experience
Schedule:
Part time
Monday - Friday
