Job Description

Company Description Rondo powers smoother clinical operations and better patient experiences by building AI agents that support care coordination teams and streamline discharge workflows. The platform predicts discharge needs from the day of admission, considering throughput and readmission risk, and coordinates placement across skilled nursing, rehab, and home health sites until medically ready patients have a confirmed bed. Rondo manages the full discharge process, including sending clinical packets, arranging transport, and keeping families and receiving teams informed, while preserving clinician control over all medical decisions. The solution integrates with existing EMRs and maintains a closed audit trail for every step, and is already live in hospitals nationwide. Rondo was built with Juxtapose, an investment firm behind dozens of healthcare and technology companies.
