Key Responsibilities
Ensure coordination with Henderson KY Ryan White Part B and Housing Program Manager and across HIV care and housing programs to deliver seamless services aligned with Ryan White Part B, HOPWA, and TBRA guidelines.
Responsible for drafting and updating departmental polices that align with funding requirements
Oversee program implementation in accordance with state and federal funding requirements, ensuring accurate service documentation, eligibility tracking, and outcome alignment.
Monitor the delivery and impact of housing services, including rental assistance, housing case management, and supportive services to promote long-term housing stability for people living with HIV.
Maintain and strengthen partnerships with health departments, housing agencies, and service providers to support referrals, shared clients, and coordinated funding efforts.
Analyze program data to track progress, identify trends, and support improvements in service delivery and client outcomes.
Maintain accurate records of programmatic MOUs, contracts, and documentation required by funding agencies across both Part B and housing programs and will ensure the objectives listed within the funding workplan for Kentucky Part B are being met.
Will participate in local planning and activity boards (for example- KHPAC, Homeless connection, etc.)
